All ABCD II five states found that efforts to help families and clinicians access resources for assessment and treatment were critical to project success. The projects undertook efforts to identify existing assessment and treatment resources, remove policy barriers to accessing those services, and facilitate referrals to these resources. These follow-up efforts helped ensure that children who are identified with potential needs receive appropriate care.
This section focuses on mechanisms to improve the ability of primary care providers to access follow-up care (including further assessment). Examples include creating a database of local and statewide resources, designating a local individual or agency that is familiar with resources for follow-up services to facilitate referrals, and working with primary care practices and representatives of local resource agencies to develop referral pathways. It includes how to use this information to identify gaps in the system and ways to fill those gaps, such as facilitating use of a diagnostic classification system specifically designed for young children.
