Through the ABCD project Vermont integrated three home visiting programs into one (Healthy Babies, Kids, and Families); the paperwork associated with assessment, referral, and monitoring was streamlined (from seven forms to one); eligibility was expanded (from 12 months to five years); and services were added to include home visiting with case management, phone consultation, targeted educational materials that highlight child development, and group education for parents and care givers.
