To make the new program feasible, the Medicaid agency collapsed certain billing codes into a new billing structure. The agency revised payment rates for home visits and established rates for phone consultation. In addition, all Healthy Babies, Kids, and Families providers in the state adopted a new form for communicating with one another. The C-Tool, as it is known, is designed to provide timely, pertinent, and non-duplicative information to participating agencies and to help ensure a more coordinated delivery of services for individual clients.
